What is the young person's birthday? (The program can include youth from 14 – 25 and will have different age groups)

What is your name and the name of the guardians for the young person? What are their relationships to the young person and their profession?

Please list everyone who is in the household. Their names, ages and professions.

Are there any other supporting family members (such as grandparents, cousins or aunts/uncles) that would assist in supporting this young person? Please list their names and relationships to the young person.

Please share some information about what you are looking for from Youth on Purpose for your young person. Include what is going on in their life that brought you to us. The more details you share the better we can support you and your young person.

We are a not-for-profit organization that exists from relying on the donations and participation of the participants to the program. What skills could you offer the program? Advertising? Venues? Social media contacts? Possible donors? Event Planning? Etc

This program requires parent/guardian participation in at least one course called “The Art of Joining a Family” Are you willing to participate in this course with your young person?
